Radiation Exposure Compensation Act (RECA)

Requirements for obtaining information from the Missouri Cancer Registry

Anyone over the age of 18 must complete their own request.

Please make sure you have all the required documents before faxing them.

If you are requesting information for yourself, the following information is needed:

  1. Please fill out and sign the Request For Information Form (include dates and type of cancer).
  2. Copy of your birth certificate or papers of adoption.
  3. Marriage license(s) documenting all of your name changes, if applicable.
  4. Copy of your driver’s license.
  5. Proof of your current address, such as an electric bill or other type of statement.

If you are requesting information for a parent, the following information is needed:

  1. Please fill out and sign the Request For Information Form (include dates and type of cancer).
  2. Copy of your birth certificate proving your relationship to the parent.
  3. Power of attorney paperwork or if deceased, the death certificate.
  4. Marriage license(s) documenting all of your name changes, if applicable.
  5. A divorce decree documenting that all marriages of the parents have ended.
  6. Copy of your driver’s license.
  7. Proof of your current address, such as an electric bill or other type of statement.
  8. If you are a stepchild of the person who became ill, send proof that their spouse was one of your natural parents and any records that show that you lived with the person who became ill in a regular parent-child relationship (for example, school records).
  9. Death certificates of any siblings who have passed away.

Once we receive your request and all the required documents, we will begin processing your request.  Please allow up to two weeks to receive a response in the mail. The response will be in the form of a letter.
